The facility failed to ensure the medication cart was locked when not under the direct supervision of a medication aide. On multiple occasions during a morning medication pass, the medication cart was observed unlocked and unattended on the 300-hallway, potentially exposing medications, including narcotics, to unauthorized access.

On 12/12/23, the medication cart was observed unlocked at four different locations on the 300-hallway while the medication aide was not present. This practice left medications, including narcotics, unsecured and accessible to others.

The facility failed to ensure appropriate referral and follow-up to meet the acute health care needs of a resident. Specifically, staff failed to notify a physician regarding a resident's non-compliance with self-catheterization, failed to follow up with a nurse practitioner or cardiologist regarding medication dosage changes, and failed to follow up on an ear infection treatment order.
