This report details the findings of a complaint investigation conducted on 01/29/2026 regarding allegations of mishandling personal belongings, mishandling pendants, and lack of dignity/respect. All three allegations were deemed 'Unsubstantiated' due to insufficient evidence to prove the alleged violations occurred.

This report details a complaint investigation regarding allegations of staff restricting resident movement. Both allegations—forcing residents to stay in rooms and preventing outings—were reviewed and subsequently deemed Unsubstantiated by the Department based on the evidence gathered during the visit.

The inspection was an unannounced Case Management - Incident visit conducted to investigate a self-reported allegation of sexual abuse. The Licensing Program Analyst interviewed the Health and Wellness Director and reviewed relevant client and staff records. Further investigation is required regarding the alleged incident.

The facility underwent an unannounced annual inspection conducted by LPAs Martha Arroyo and Valeria Conway. The inspection covered resident rooms, kitchen operations, common areas, and record reviews. All observed areas, including hot water temperatures and record keeping, were found to be in compliance with regulations.
