Key Review Excerpts

My Mother recently spent some time here and all of the staff was absolutely amazing. I live in another state and the staff gathered regularly to reached out to me to keep me updated.

Long-term resident's family · 2022★★★★★

The left hand doesn’t know what the right hand is doing, lack of communication from the top down, No one wants to take responsibility for anything, always passing the buck. Terrible follow through on anything in question. I will say that the oral, occupational and physical therapists are the best part of that place.

Family member · 2020☆☆☆☆

My sister in law was placed in the hospice unit. Shared a bathroom with 4 patients. Was a fall risk . fell 9 times. No poting on her door that she was at risk. Facility is filthy.

Brookhaven Nursing & Rehab shows significant ongoing concerns with families filing multiple complaints triggering investigations. Medication management problems persist across surveys, with deficiencies in 2020, 2023, and 2024 for medication errors and pharmaceutical services. The facility also has recurring issues with infection control, food safety, and abuse prevention. While all violations show correction dates, the pattern of repeated medication safety problems and complaint-driven investigations warrants careful consideration during any visit.
