A relicensure survey was completed on 10/15/24. Deficiencies were cited. Based on observation and interview the residence failed to provide a physically safe environment including, but not limited to, measures to reduce the risk of potential hazards in the physical environment related to the unique characteristics of the population, affecting seven sample residents. Findings include:During an environmental tour on 10/15/24 at approximately 8:00 a.m., the residence' s surrounding paved walkway was uneven. Multiple areas of the walkway had one to two inches of raised surfaces from the midpoint, creating a tripping hazard. On 10/15/24 at approximately 1:35 p.m., the administrator acknowledged the uneven surfaces and stated that the residence' s outdoor walkway was unsafe. THIS PORTION OF THE REPORT IS FOR INFORMATIONAL PURPOSES ONLY. No response is necessary.The residence was advised it must review and maintain the following processes in accordance with existing program regulations found at 6 CCR 1011-1, Chapter 7.10.1 The assisted living residence shall have readily available a roster of current residents, their room assignments and emergency contact information, along with a facility diagram showing room locations.12.9 The comprehensive assessment shall be updated for each resident at least annually and whenever the resident' s condition changes from baseline status.12.10 Each resident care plan shall: (B) Reflect the most current assessment information12.19 The assisted living residence shall encourage residents to maintain and develop their fullest potential for independent living through individual and group engagement opportunities.14.31 The administrator and the QMAP supervisor shall, on a quarterly basis, audit the accuracy and completeness of the medication administration records, controlled substance list, medication error reports, and medication disposal records. Any irregularities shall be investigated and resolved. The results of the audits shall be documented and routinely included as part of the assisted living residence' s Quality Management Program assessment and review.18.4 ..
