Specifically, the resident's records lacked the results of a TB test and the facility could not provide evidence of a valid 2-step test. The Administrator incorrectly believed a chest x-ray could be used as a substitute for the required TB skin test.

The facility failed to ensure the accuracy of medication administration records (MAR) for three sampled residents. Specifically, the electronic medication administration records contained duplicate entries for medications administered, such as Rybelsus, Divalproex Sodium, and Metformin.
