Key Review Excerpts

Call lights were consistently ignored; on one occasion, I visited and found her with dried vomit on her clothing. She stated she had been waiting for assistance for over an hour.

Memory care family member · 2025☆☆☆☆

When released, the family discovered that he had a large, untreated and open wound on his tailbone, one on his heel and one on his leg. There was never any mention of any sores or wounds on him from any of the staff even though we were in constant contact with them.

Long-term resident's family · 2022☆☆☆☆

She would call for help and literally wait 45 -75 minutes for someone to come in and help her go to the bathroom. Several times she had accidents in bed because she had to wait so long.

17 complaint-triggered

Bradford Place Care Center has faced multiple complaint-triggered investigations (11 total), with families reporting concerns about medication management, pressure ulcer care, and resident rights issues. The facility shows recurring problems with medication safety, building safety systems (fire alarms, electrical), and infection control across multiple surveys from 2018-2024. While all deficiencies show correction dates, the pattern of repeated violations in key care areas suggests ongoing challenges with consistent quality management that families should carefully evaluate.
