The facility failed to ensure medication administration was consistent with current physician orders. Specifically, Resident #2 was being administered fluoxetine 40mg despite a physician order dated 09/22/21 decreasing the dose to 20mg. This resulted in the administration of an incorrect dosage based on the most recent medical instructions.
