The facility failed to ensure a resident received prescribed medication to treat pain and stiffness. Specifically, cyclobenzaprine 10mg was not available at the facility from 09/20/24 to 10/09/24, despite the Medication Administration Record (MAR) being documented as administered during that period. The Administrator admitted to documenting administration in error and was unaware the medication had run out.

The facility failed to ensure that medication administration was in accordance with physician orders. Specifically, Resident #2 was not provided with their prescribed cyclobenzaprine for pain and stiffness from 09/20/24 through 10/09/24 because the medication was unavailable and no refill had been ordered.

The facility failed to ensure that a resident had a required annual medical examination (FL-2) completed. A review of Resident #1's records showed their last examination was dated 02/17/22, which was past the annual requirement. Additionally, facility leadership was unaware that these examinations must be updated annually.

The facility failed to ensure that one of two sampled residents had a completed FL-2 medical examination performed annually. The facility failed to accurately document medication administration on the Medication Administration Record (MAR) for one resident. Specifically, there were duplicate entries for an antacid at both 8:00am and 6:00pm, and the 8:00am time had been handwritten to change to 6:00pm without resolving the conflicting entries.
