A relicensure survey was completed on 12/3/24. A deficiency was cited. Based on observation, record review, and interview, the residence failed to review and update the resident agreement annually to reflect its current licensure status and changes to services provided, affecting seven current residents.Findings include:1. ObservationsOn 12/3/24 at approximately 7:35 a.m., the front door of the building was observed to be locked and staff assistance was required before entry. A number combination for the use of a wanderguard was posted on the wall in the kitchen area. An egress alert was also observed on the north-east exit door of the residence.2. Record Review The residence' s resident agreement policy failed to identify the residence as an assisted living residence (ALR) The document also failed to state, in clear terms, whether the residence was providing services as a secure or unsecure environment.A resident agreement, dated on 5/10/24 and signed by Resident #1, did not clearly state whether the resident received services in a secure environment. 3. Interviews On 12/3/24 at 3:45 p.m., the administrator stated she was aware that the residence was required to review the resident agreement annually and update it as necessary. She acknowledged that the residence had not updated the resident agreement to reflect the residence' s current licensure status.On 12/3/24 at 3:55 p.m., the assistant administrator and the purchasing coordinator stated the residence ceased to be a secure environment in May 2022. They added that the code combination for a wanderguard located in the kitchen area was deactivated and needed to be removed. They said they needed to review and update the resident agreement to reflect the residence' s current licensure status.
