Key Review Excerpts

My wife was admitted to Bear Mountain for short term Rehab for a broken ankle after surgery. The 1st and only night she was there, she needed to go to the bathroom. She pressed the nurse call button many times to no help or response.

I’m at Bear Mountain Rehab facility and all I can say is I’m so BLESSED to be working with my PT team that consist of Karen, Julie and Morgan. Shortly I believe I’ll be working with Lindsay. My team is absolutely amazing.

Rehab patient · 2024★★★★★

I spent approximately 5 weeks at Bear Mountain Sudbury. I came from a long term hospital stay, so I was ready in mind , but not in body to go home. To my complete surprise from the top ( the administrator) down to the lovely people who kept the rooms clean, it was a wonderful experience.

Bear Mountain at Sudbury shows ongoing quality concerns with families having filed complaints about nutrition, resident rights, and care planning issues. The facility has persistent problems with resident rights and communication, medication management, and infection control that appear across multiple surveys from 2022 through 2025. While all deficiencies show correction dates, the recurring nature of issues in core care areas suggests systemic challenges that families should carefully consider before placement.
