Resident A's medication administration record (MAR) for Haloperidol lacked detailed information in the service plan regarding how agitation is demonstrated, what behaviors require medication, or if nonpharmaceutical interventions can be used. Similar findings were noted for Resident C and Resident D.
The kitchen manager had no record of the last time the dishwasher heat sanitation was tested and reported that routine checks are not completed, which fails to protect residents from potential infection from machine malfunction.
The dry storage area in the kitchen contained items that were opened, unsealed, and not dated (including cereal, brown sugar, and nuts).
Resident B's service plan lacked detail regarding specific needs; it did not specify which hospice agency was involved in her care, and it did not define the type of assistance required for her oxygen use.
No record of staff training was found in the employee record for staff person 1 (SP1).
