The facility failed to ensure necessary referral and follow-up to meet the acute health care needs of a resident. Specifically, the facility did not adequately follow up on a physician's order for a hospital bed, with no documentation of attempts to obtain approval between June and July 2025. The resident did not receive the required hospital bed until the day of the survey.

The facility failed to administer Clonidine as ordered by the primary care provider for 10 out of 11 identified opportunities over a three-month period for one resident. Specifically, the facility did not administer the medication when the resident's blood pressure readings met the prescribed thresholds for intervention.

The facility failed to ensure that three sampled staff members (Staff A, B, and C) had criminal background checks on file in accordance with G.S. 114-19.10 and 131D-40. While consent forms for background checks were present in the personnel files, the actual documentation of the completed criminal background checks was missing.

The facility failed to ensure that all staff members had a criminal background check on file in accordance with state requirements. Specifically, a review of personnel files for three sampled staff members revealed no documentation of completed criminal background checks.
