The facility failed to ensure that one sampled staff member had a North Carolina Health Care Personnel Registry check completed prior to being hired. The registry check was not performed until several days after the employee began working.
The facility failed to ensure that one sampled staff member had a criminal background check completed upon hire. While a consent form was present in the personnel file, there was no documentation showing the check was completed before the staff member was allowed to work.
