This report details an investigation into complaints regarding resident care, including delayed response to calls for assistance, soiled diapers, and transfer assistance. The investigator found that the allegations could not be substantiated due to a lack of preponderance of evidence.

The facility underwent a required annual inspection on November 14, 2024. The Licensing Program Analyst observed no issues or concerns during the tour and review of documentation. The facility appeared to be compliant with safety and operational standards.

The annual inspection was conducted on November 29, 2023. The facility was generally found to be clean, in good repair, and operating in safe conditions. However, one deficiency was noted regarding the facility operating below its licensed capacity.

The case management visit identified deficiencies related to staff training. Specifically, the CPR/First Aid certifications for two staff members were found to have expired on March 9, 2023. The facility was cited for not meeting the requirement that staff provide appropriate first aid training.

The facility underwent an unannounced annual required licensing inspection. The Licensing Program Analyst conducted a thorough inspection, focusing on infection control protocols, and found no deficiencies during the visit. The licensee was advised to submit a current Personnel Report (LIC 500) within 10 business days.
