Questions for Your Tour

  • 1Given that rehabilitation is a noted strength here, could you explain how your therapy team coordinates with the nursing staff to ensure a smooth transition for residents recovering from surgery?
  • 2With the facility maintaining a 3-star overall CMS rating, what specific steps is leadership taking to address the areas that currently prevent a higher rating?
  • 3I noticed some feedback regarding response times; what is your current protocol for ensuring residents receive timely assistance when they press their call buttons, especially during shift changes?
  • 4Maintaining high hygiene standards is very important to our family; could you walk us through your daily routine for room cleaning and personal care assistance?
  • 5I see that you occasionally respond to online feedback; how does that process help your team improve the daily experience for residents?
  • 6Could you share a sample calendar of your activities program and tell us how you encourage residents who might be hesitant to join in?

Key Review Excerpts

The staff is amazing. Each and everyone from the reception desk to the nurses station and social services are upbeat, helpful and dedicated to elder care. Very clean facility. Doesn't have an odor.

Memory care family member · 2026★★★★★

Most of the staff are genuinely kind and caring — they take time to get to know the residents and make sure they feel comfortable and cared for. You can really tell they love what they do.

Long-term resident's family · 2025★★★★★

The staff was friendly but they are like most places that are taking advantage of Medicare. Mom had speech therapy added in even though she didn’t need it. Also they are not attentive, let mom pee herself after multiple requests for help.

Families have filed complaints that triggered inspections finding deficiencies in food safety and accident prevention. The facility shows recurring issues with fire safety equipment, particularly smoke-blocking doors cited twice across different surveys. Other problems have included medication errors, abuse reporting delays, and building maintenance issues. All deficiencies have correction dates, but the repeat fire safety violations suggest ongoing facility maintenance challenges that warrant attention during any visit.
