The facility failed to ensure that a relief Supervisor-in-Charge (Staff C) had a completed Medication Administration Skills Validation form on file for this specific facility. Although the staff member had a valid medication test from 2012 and a validation form from a different facility, the required documentation for this location was missing.

The facility failed to maintain a three-day supply of perishable food for its census of five residents. Observations and interviews revealed expired milk and eggs, as well as instances where staff had to borrow food from sister facilities to ensure residents were fed.

The facility failed to provide adequate supervision for a resident with a history of smoking inside the facility. Evidence included the smell of smoke in the resident's room and hallway, as well as staff providing the resident with cigarettes despite the resident's known smoking violations. Furthermore, the resident's care plan and smoking evaluation documentation were incomplete or failed to address the need for increased supervision.
