The facility failed to notify the local county Department of Social Services (DSS) regarding two separate incidents involving Resident #1. Specifically, the resident required emergency medical treatment for a scalp laceration on 01/03/23 and a fractured humerus on 01/20/23, but DSS was not notified of these injuries.

The facility failed to ensure medications were not pre-charted prior to administration and failed to observe a resident actually taking their medication. During an inspection, a medication cup containing levothyroxine and omeprazole was found in a resident's room without the resident or staff present, despite the eMAR being documented as administered at 7:00am. The Medication Aide admitted to leaving the room before the resident took the medication and noted that they do not always observe residents taking their medications.

During an inspection, a medication cup containing unadministered capsules and tablets was found in a resident's room, while the electronic record indicated the medications had already been administered earlier that morning.

The facility failed to ensure a staff member was available at all times, leaving residents without accessible supervision. Interviews revealed periods of up to one hour where staff were unavailable, causing resident anxiety and leaving residents unable to access medication or assistance during emergencies.
