The facility failed to ensure that a staff member (Staff B) who had worked at the facility for more than six months had completed the required 25-hour personal care training program. There was no documentation available to verify the completion of this mandatory training.

The facility failed to ensure a physician was contacted when Lovenox was not administered as ordered and when Pyridium was unavailable for a resident. Specifically, there was a gap in the administration of Lovenox from 11/19/15 to 11/30/15 without documented physician notification.

The facility failed to ensure that a staff member (Staff B) who had been employed for more than six months had completed the required 25-hour personal care training program. Personnel records showed no documentation of training completion, and the Administrator could not confirm the staff member had received the necessary instruction.
