A re-licensure survey was completed on 11/12/24. One deficiency was cited. 14.7 The assisted living residence shall ensure that each resident receives proper administration and/or monitoring of medications.25.26 A secure environment shall meet the following criteria: 5) Has a fence or enclosure around the perimeter of the outdoor area that is no less than six (6) feet in height and constructed to reduce the risk of resident wandering or elopement from the area. (a) If the fence or enclosure has gated access which is locked, all staff assigned to the secure environment shall have a readily available means of unlocking the gate in case of emergency. Based on observation, record review, and interview, the residence failed to prohibit a qualified medication administration person (QMAP) from assessing the need for oxygen use, affecting two of three current residents (#1, #3).Resident #3 was admitted to the residence on 8/12/22 with a diagnosis of dementia. During an environmental tour of Resident #3' s room on 11/12/24 revealed on oxygen concentrator. A written practitioner order, dated 8/8/24 directed the residence to administer oxygen via nasal cannula at two liters per minute, as needed for hypoxia. The care plan for Resident #3, dated 6/21/24 read in part, "Unlicensed assistive personnel to assist residents with oxygen, check oxygen rate, assist to place oxygen cannula". On 11/12/24 at 1:21 p.m., the power of attorney for Resident #1 stated the residence used a pulse oximetry to determine when Resident #1 needed supplemental oxygen. On 11/12/24 at 1:45 p.m., Staff #4 stated Resident #1 was assisted with the use of oxygen during nighttime sleeping hours and every evening. On 11/12/24 at approximately 2:00 p.m., the administrator stated they were aware staff were assisting Resident #3 with the use of oxygen during the evening hours. Similar deficient practice was found for Resident #1.
