A relicensure survey was completed on 12/10/24. A deficiency was cited. Based on interview and record review, the residence failed to ensure resident' s medication administration record (MAR) contained accurate information, affecting two of three sample residents (#1, #3). Findings include:1. Residence PolicyThe residence' s Medication Administration policy, dated 2024, read in part: "Each qualified medication administration person, nurse, or authorized practitioner shall document accurate information in the medication administration record including any medication omissions, refusals, and resident reported responses to medications."2. Resident #3 was admitted to the residence on 3/21/24.a. AmoxicillinA written practitioner' s order, dated 11/26/24, directed the residence to administer amoxicillin 875 mg twice daily for five days. However, the December 2024 MAR read the resident refused two doses of the medication on 12/2/24 and one dose on 12/3/24. However, the resident did not refuse the medication was not refused, staff did not administer the medication because it was discontinued.b. MucinexA written practitioner' s order, dated 11/25/24, directed the residence to administer Mucinex 600 mg twice daily for seven days. However, the December 2024 MAR read the resident refused the medication on 12/2/24 in the evening. However, Staff #3 inaccurately documented the medication as refused although it had been discontinued. 3.. (A) All medication stored in a refrigerator shall be clearly labeled with the resident' s name and prescribing information.
