Provider failed to protect client from harm; a staff member fell asleep, allowing the client to access car keys, take a staff vehicle, and get into a collision, resulting in injury and legal/financial hardship for the client.

Provider failed to implement the Individual Instruction and Support Plan (IISP) as evidenced by the client eloping six times between 2023 and 2024, culminating in a vehicle collision.

Findings regarding elopement and physical hold were removed; door alarm location updated from inside to outside.
