Assisted Living at Spring LLC

A relicensure survey was completed on 3/3/26. No deficiencies were cited. THIS PORTION OF THE REPORT IS FOR INFORMATIONAL PURPOSES ONLY.No response is necessary.The residence was advised to review and maintain the following processes in accordance with the existing program regulations at 6 CCR 1011-1, Chapter 7.(U1600) 14.29 All prescribed and PRN medications shall be listed and recorded on a medication administration record (MAR), which contains the name and date of birth of the resident, the resident ' s room location, any known allergies, and the name and telephone number of the resident ' s authorized practitioner.(B) As part of the medication administration record, the assisted living residence shall maintain a legible list of the names of the persons utilizing the record for medication administration, along with each of their signatures and, if used, their initials.(U2510) 21.1 The assisted living residence grounds shall be kept free of high weeds, garbage, and rubbish.(U2744) 22.43 Power strip surge protectors are permitted throughout the assisted living residence with the following limitations:(C) Power strips shall not be linked together.

A relicensure survey, with complaint #CO29518, was completed on 5/31/23. A deficiency was cited. Based on observation and interview, the residence failed to ensure designated smoking areas were equipped with fire resistance wastebaskets and resident rooms occupied by smokers had fire resistant wastebaskets, affecting 15 of 15 sample residents who smoked (#2-#16).Findings include:An environmental tour of the residence on 5/31/23 from 10:30 a.m to 11:00 a.m., revealed Residents #2-#16 did not have fire resistant wastebaskets. On 5/31/23 at 7:53 a.m., the administrator stated Residents #2-#16 smoked.During a second interview on 5/31/23 at 11:00 a.m., the administrator stated she was aware the 15 smoking residents were required to also have fire resistant wastebaskets in their rooms. However, she stated since the wastebaskets were made of metal and she thought they were fire resistant.
