The facility failed to ensure that one of three sampled residents had a functional assessment (FL2) completed annually. Specifically, Resident #1's assessment had not been updated since November 2022, and there was incorrect documentation suggesting an annual update was unnecessary due to the resident being private pay.

The facility failed to revise the resident care plan as needed based on further assessments of the resident according to regulatory requirements.
