Key Review Excerpts

I am now fully recovered from my fractures; walking without pain; doing my own laundry; taking my own showers; and participating and all my therapy sessions. Without the therapy team, I would not be where I am today.

Rehab patient · 2026★★★★★

The huge spectrum possible outcomes upon using a call button seems ever more infinite. From vibrant go getters with knowledge and caring attitude to bully tactics to intimidate patient into not making additional requests.

Family member · 2023☆☆☆☆

My mother entered your facility for cardiac rehab following her open heart surgery. She entered into the facility walking, talking and able to handle her own basic 'bodily' needs. Two weeks later my mother could no longer talk, walk, and wearing pampers, had to be removed and emitted into UVA hospital for treatment and care.

34 complaint-triggered
$63,294 in fines

Most concerning is the pattern of 34 complaint-triggered deficiencies, indicating families have repeatedly filed reports about care issues. The facility shows persistent problems with care planning and assessment, nutrition and dietary services, and medication management across multiple surveys from 2021 to 2025. While all deficiencies show correction dates, the recurring nature of similar issues suggests ongoing quality challenges families should carefully evaluate.
