FACILITY EVALUATION REPORT

The inspection was an unannounced annual visit conducted by Marcella Tarin. The facility was generally observed to be clean and safe, with proper procedures noted for fire safety and food storage. However, one critical deficiency was cited regarding the unsecured storage of resident medications, posing an immediate risk to the resident in question.

This was a Case Management follow-up visit conducted to review deficiencies cited previously and to investigate a self-reported medication error from 7/30/2025. Although the facility was cited with a Type A deficiency on 9/11/2025, the report notes that the deficiencies were cleared during today's visit. No new deficiencies were cited during the current visit.

The inspection was a case management follow-up regarding a resident's elopement. Multiple serious deficiencies were cited, including a failure to ensure safe accommodations for residents with neurocognitive disorders and wandering behaviors. The most critical finding was the resident eloping from the facility and sustaining injuries, leading to an immediate civil penalty assessment.

The facility underwent a Case Management-Incident visit following an incident report. The Licensing Program Analyst conducted a tour and interviewed staff and a resident. No deficiencies were cited during this visit per California Code of Regulations (CCR) Title 22.

The facility underwent a Required - 1 Year inspection. The Licensing Program Analyst noted that all exits and passageways were clear of obstruction, and the facility appeared generally compliant. Specifically, the report states that no deficiencies were cited during this visit per California Code of Regulations Title 22.
