These current public-data fields deserve follow-up with the facility and the official Medicare record. They are not a clinical risk score or a substitute for an in-person assessment.
Accordius Health at Charlotte has a middle-range Medicare overall rating. Strengths include effective physical and speech therapy teams. Some reviewers note concerns about neglect of basic hygiene and toileting needs (mentioned by 10 reviewers). Review the health-inspection, staffing, and quality-measure components separately, then visit in person.
Accordius Health at Charlotte (now operating as Redwood Health and Rehabilitation) receives highly polarized feedback, with many families reporting severe neglect, poor communication, and hygiene issues, while others praise the therapy team and specific staff members. The facility has faced recurring allegations of call bells being ignored, patients being left in soiled conditions, and a lack of consistent medical oversight. Prospective families should be aware of the stark contrast between the positive experiences of some rehab patients and the distressing reports of long-term care neglect.

Key Review Excerpts
“My father was there for 6 days, it took us a matter of 2 hours to realize that was not a place for anyone! He was there for rehab and PT worked with him 1 day. His call bell was broken and they gave him a cow bell, when he rang it no one come.”
“My uncle has been there for almost 2 and 1/2 years now, I remember when he first came there how he was pretty much a recluse and could not walk. But now he is a totally different guy & next week he's expected to be getting his own apartment and walk right out of the door.”
“My mom was admitted here August 12th 2024 , she caught a bed sore and had a staff infection Due to neglect from turning her, so she got a bad bed sore that got infected, and it infected Her Blood stream bed sore was so big you could put your fist through it.”
