A relicensure survey was completed on 1/16/25. A deficiency was cited. Based on record review and interview, the residence failed to ensure a name-based criminal history check conducted by the Colorado Bureau of Investigation (CBI) was completed for each prospective staff member prior to staff hire, affecting six current residents.Findings include:On 1/15/25, documentation of dates of hire provided by the administrator read that Staff #1 had a hire date of 8/15/24.Staff records for Staff #1 revealed no documentation that the residence completed a name-based criminal history check conducted by the CBI prior to her hire date.On 1/15/25 at approximately 2:45 p.m., the administrator stated all required background checks were conducted through an outside agency. She was unaware if the background check was conducted by the CBI.On 1/15/25 at 4:00 p.m., the owner of the background check company stated he could not provide documentation that a name-based history check was done through the CBI.On 1/15/25 at 4:15 p.m., the administrator stated she did a national background check but could not provide a name-based criminal history check conducted by the CBI for Staff #1. She acknowledged the regulation required a CBI and that she could not provide documentation.

A recertification survey was completed on 11/14/23. A deficiency was cited. Based on interview and record review, the facility (residence) failed to maintain and follow written policies and procedures for the administration of medication in accordance with 6 CCR 1011-1, Chapter VII Medication Administration Regulations, affecting two of three sample participants (residents) (#2, #3).Findings include:1. Chapter VII regulations governing assisted living residents part 14.29, requires all prescribed and PRN medications shall be listed and recorded on a medication administration record (MAR) which contains the name and date of birth of the resident, the resident' s room location, any known allergies, and the name and telephone number of the resident' s authorized practitioner. (D) Each qualified medication administration person, nurse, or authorized practitioner shall document accurate information in the medication administration record including any medication omissions, refusals, and resident reported responses to medications. The residence' s Medication and Medication Administration policy, dated January 2019, read in part: "each qualified medication administration person (QMAP)... must accurately document each medication or monitoring event at the time the event is completed for each resident. (QMAPs)... are required to document accurate information ... including any medication omissions."a. Resident #2 was admitted to the residence on 5/26/23 with a diagnosis of epilepsy. SeroquelA written practitioner order, dated 8/25/23, directed the residence to administer seroquel 200 mg three times daily. However, the October 2023 electronic medication administration record (eMAR) contained a signature with a circle on 10/8/23 for the morning and afternoon dose. However, there was no reason given as part of the eMAR as to why the medication administration events were circled. Additionally, documentation on the eMAR at 10:00 a.m. on 10/7/23 read seroquel was not available to be administered on 10/7/23 in the morning, afternoon and evening dose.
