What this means for your family

Yardley offers an excellent therapy program that can significantly aid in physical recovery, and their concierge team is highly effective at assisting with daily needs. However, families should be aware of significant communication gaps regarding medical appointments and discharge planning; we strongly recommend maintaining frequent, direct contact with the social worker and nursing management to ensure your loved one's care plan is followed.

Key Review Excerpts

The entire staff, especially my therapist Manisha Shah was very helpful and caring. She took a lot of her time to make sure I understood my exercises and made sure they were done correctly.

Rehab patient · 2026★★★★★

The nurses are stretched thin, and I, with a reasonably high brain function, caught at least four medication errors in the course of my stay.

Rehab patient · 2026★★★☆☆

The administrative staff needs the be on the firing line. They make doctors appt but they don't make sure the patient goes. My mom had 3 appointments that she desperately needed and she missed them all and they discharged her the next day.

5deficiencies
Well below state avg (10.1)
5 complaint-triggered

Families have filed complaints leading to deficiencies, with the most recent in January 2025 regarding proper notification before transfers that remains under correction. The facility shows recurring issues across multiple surveys in quality of care (including accident prevention, daily living assistance, and pain management), fire safety systems, and nursing staffing transparency. While all deficiencies have correction plans, the pattern of repeated problems in resident care and the recent complaint-triggered violations suggest ongoing operational challenges families should carefully evaluate.
