What this means for your family

Wooded Glen offers a beautiful environment and excellent rehab services, but there is a clear inconsistency in daily care standards. When touring, we strongly recommend asking for specific protocols regarding call light response times and how the facility ensures residents receive adequate assistance with meals and personal hygiene.

Wooded Glen receives polarized feedback, with many families praising the facility's hotel-like aesthetics, friendly staff, and high-quality rehab services. However, a significant number of reviewers report serious concerns regarding neglect, including slow response times to call lights, inadequate assistance with feeding and hygiene, and recurring issues with bedsores. Prospective families should weigh the positive, warm atmosphere against these recurring reports of understaffing and inconsistent care.

Questions for Your Tour

  • 1I noticed your facility has a beautiful, hotel-like atmosphere; could you walk me through how your team ensures that same level of attention to detail when it comes to daily hygiene and personal care assistance?
  • 2Given that your staffing rating is a 3/5, what specific protocols do you have in place to ensure that call lights are answered promptly and that residents receive timely assistance during meal times?
  • 3I appreciate that your team is active in responding to feedback online; how do you typically communicate with families if a concern arises regarding a resident's physical health, such as skin integrity or bedsore prevention?
  • 4With your strong focus on rehab and physical therapy, how do you balance those intensive programs with opportunities for social engagement and daily activities for residents who may not be in therapy?
  • 5Could you explain the process for how your nursing staff monitors residents throughout the night to ensure their needs are met, especially when they require assistance with repositioning or personal care?
  • 6Since you have a 5-star rating for health inspections, what are some of the internal quality checks you perform to maintain those high standards while managing the current staffing levels?

Key Review Excerpts

Every time we visit they have wet pants and seems like a new bed sore. The meals have been cold and the room doesn't appear to never be cleaned well. On a visit I've experienced the call light being on over 20 mins.

Family member · 2023☆☆☆☆

The staff has been very caring and attentive to her. Every time I come in, someone shares a story of how she is adjusting. Even the residents have gone out of their way to welcome her.

Memory care family member · 2025★★★★★

Call lights not being answered for longer than 25 minutes. Meals not delivered to the patient rooms. So mom was offered a peanut butter and jelly sandwich for dinner. This happened several times.

5 complaint-triggered

Wooded Glen has concerning patterns with multiple families filing complaints that triggered federal inspections, resulting in 5 complaint-driven deficiencies. The facility shows recurring problems with medication management, safety violations, and infection control across multiple surveys from 2019 to 2025. While the facility reports correcting violations, the persistence of medication errors and safety issues through recent complaint investigations suggests ongoing care quality concerns that families should carefully consider.
