What this means for your family

This facility shows a stark contrast between its well-regarded rehab therapy department and its long-term care unit. Families should be extremely cautious; if you choose this facility, perform unannounced visits at various times of the day and night to monitor hygiene and staff responsiveness, and ensure you have a clear, documented plan for how medical updates will be communicated to you.

Questions for Your Tour

  • 1Given the current staffing levels, what is the average response time for a resident using their call light, and how do you ensure needs are met during shift changes?

  • 3How do you manage communication with families regarding changes in a resident's medical status or medication adjustments to ensure we are always kept in the loop?
  • 4I see that you are very active in responding to feedback online; how do you use that family input to improve the quality of meal services and dining experiences for the residents?
  • 5Since the therapy team is so highly regarded here, how do you integrate their work with the nursing staff to ensure residents maintain their progress throughout the day?

Key Review Excerpts

My grandmother wasn’t even in their care for 2 weeks before she had to be taking to the hospital. When she arrived, she was malnourished. Dehydrated had not had her medication in days. She had a bacteria infection from them, letting her sit in her own waste and not changing her.

Family member · 2024☆☆☆☆

I can only speak for the Rehab part of the facility. Nursing staff is horrible. Can't get meds in a timely manner. Food is the worse. I am not a difficult person. I work in Healthcare. I understand people call off, but when I ask for a pain pill at 5pm and don't get it till 10pm that is unacceptable.

Rehab patient · 2025☆☆☆☆

The nursing home at WRV has done an excellent overall job of caring for one of our family members for over 5 years. The overwhelming majority of the staff is superb - that includes nurses aides and nurses, doctors, receptionists, and a variety of other staff members.

Recent family complaints triggered serious deficiencies in nursing staffing, medication management, and resident safety, with some issues remaining uncorrected. This facility shows recurring problems across multiple areas including inadequate nursing staff levels, medication errors, abuse prevention failures, and poor care planning that persist across surveys from 2018 to 2025. While most deficiencies are eventually corrected, the pattern of repeated violations in critical care areas suggests ongoing operational challenges that families should carefully consider.
