Key Review Excerpts

Dedicated care, but high turnover of CNAs, and short staffing. Nursing and admin staff very dedicated to the patients there. Excellent and very clean facility.

Family member · 2019★★★★

This place was not taking care of my Dad properly doctor said not being cleaned properly and not receiving optimal care. He is in the hospital again less than a month later.

Family member · 2022☆☆☆☆

Food for the member is not cooked on site. It is brought in from the King Va which is about 2 hour away from Union Grove. Once a day. So it sits from the time it is cooked at the King Va to the time each meal is served.

$125,470 in fines

This facility has concerning patterns of recurring violations, particularly in three key areas: protection from abuse and neglect, accident prevention and safety supervision, and proper reporting of incidents to authorities. With 76 total deficiencies and 32 complaint-triggered violations showing families have filed reports, the facility has faced repeated issues across multiple surveys. While all violations show correction dates, the persistent recurrence of similar problems, especially around resident safety and incident response, suggests ongoing systemic challenges that families should carefully consider.
