Questions for Your Tour

  • 1I noticed your team is very active in responding to online feedback; how do you use that family input to improve daily operations?
  • 2With the rehabilitation team being a highlight here, how do you ensure that same level of dedicated attention carries over to daily hygiene and personal care needs?
  • 3Could you walk me through the protocol for call lights, especially during weekend shifts when staffing levels might fluctuate?

Key Review Excerpts

The therapy department was GREAT. The nursing department was very attentive when she would ring her light for assistance. Overall a wonderful facility!

Long-term resident's family · 2024★★★★★

My father has a form of lewy body dementia. Antipsychotics medications such as Haldol are contraindicated in this population. It did not stop Valley Health from giving it to him in an effort to medicate him to the point where they would not have to take care of him.

Memory care family member · 2022☆☆☆☆

I popped into see Mom on occasions. There was human feces all over bathroom floor that been there for days. It's very hard to get through to talk to Mom. They lost her dentures.

Valley Rehabilitation and Nursing Center has accumulated 44 deficiencies across three surveys, primarily involving fire safety systems, exit clearance, and care planning. The facility shows recurring issues with fire safety equipment maintenance, emergency preparedness, and resident assessment processes that persist across multiple inspection periods. While all deficiencies report planned corrections, the pattern of repeated fire safety violations and care planning gaps suggests ongoing operational challenges families should investigate during visits.
