What this means for your family

While the therapy department has historically been a strength, the facility has seen a sharp decline in care standards recently. Families should conduct an unannounced visit to check for cleanliness and observe staff-to-resident interactions, specifically paying attention to the condition of linens and resident hygiene.

Key Review Excerpts

The smell of urine and musty moldy smell the second I walked into the building gave me a headache. We walked into my grandpas room and they had 2 other patients in a tiny room. It also smelt like poop.

Memory care family member · 2026☆☆☆☆

My love one care is not at a 4 or 5 care level. He doesn't get the proper care because the facility is short staff. The facility is very clean. Since I complained about my love one care ,about him not being washed, bed linen is not being change.

Long-term resident's family · 2024☆☆☆☆

Amazing therapy programs at Twin View. Helen, Tiffany, and Wendy are wonderful to work with.

Clean record — no deficiencies

Twin View Health and Rehab shows recurring issues across all three surveys, primarily with fire safety systems, food handling and storage, and emergency preparedness. The facility has had repeated problems with fire alarm maintenance, sprinkler system inspections, and food safety protocols spanning from 2020 to 2024. While all deficiencies show correction dates, the pattern of similar violations reoccurring suggests ongoing challenges with maintaining consistent safety standards.
