FACILITY EVALUATION REPORT

The unannounced Annual Required inspection visit was conducted on 03/11/2025. The facility was found to be generally compliant, with positive observations regarding food storage, room condition, and safety equipment. No deficiencies were cited during this inspection.

The inspection was conducted unannounced to continue an annual required inspection. The LPA reviewed staff and resident files, and the administrator's certification was found to be up to date. No deficiencies were cited during the inspection, though the administrator was asked to submit several updates by 05/29/2024.

The facility underwent an Annual Required inspection conducted by the LPA. The initial observations noted that the facility was comfortable, passageways were clear, and essential supplies were available and stored properly. No deficiencies were cited during the inspection.

This report details two separate complaint investigations conducted on the facility. In the first investigation regarding resident discharge threats, the complaint was found to be unfounded. In the second investigation concerning the physical plant, the allegation was deemed unsubstantiated. No deficiencies were cited in either investigation.

The facility was inspected for its annual required visit. While the general environment was found to be clean and many areas were compliant, two deficiencies were noted. These include an overdue fire extinguisher charge and an expired administrator certificate that requires immediate updating.

The inspection was a Case Management follow-up regarding a self-reported fall incident. The Licensing Program Analyst reviewed discharge documents and was informed that the resident was brought back to the facility the same day with no reported concussion or changes in care. No deficiencies were cited during this inspection.

The facility underwent an unannounced post-licensing infection control inspection. The inspector noted that the facility was clean, and several required items, such as fire extinguisher charging dates, were observed. No deficiencies were cited during this inspection.
