FACILITY EVALUATION REPORT

The facility underwent a Case Management inspection, which was a follow-up to an annual inspection conducted on 7/22/2025. The inspector observed no immediate health or safety hazards, noting the facility was clean, well-lit, and at a comfortable temperature. The facility was found to be compliant with observed standards.

The facility underwent an Annual Required - 1 yr. visit and was generally found to be clean with appropriate safety measures in place for many areas. However, two deficiencies were cited: the main shower area lacks required slip-resistant flooring, and five window screens, including four in resident bedrooms, were missing.

The inspection identified multiple deficiencies related to sanitation and maintenance across several areas of the facility. Specific issues included plumbing leaks, visible biological/discoloration buildup in bathrooms, and evidence of rodent droppings in the kitchen. All cited deficiencies were classified as Type B (non-compliance).

The facility underwent an unannounced annual inspection. While the centrally stored medication record was found to be complete and accurate, a deficiency was noted regarding medication preparation. Specifically, medications were found to be pre-poured, violating regulations concerning medication container transfers.

However, one deficiency was noted regarding the fire extinguisher's last charging date.
