Hazardous and toxic materials not stored in a safe manner: disinfectant left unattended; easy-open sharps container and Lubriderm left unattended in resident room; oxygen tanks left free standing without means to prevent falling.
Resident rooms had doors leading to an enclosed courtyard with slide-bar toggle locks that would prevent residents from accessing the courtyard.
Posted menus in assisted living area did not include all regular and therapeutic diets being served.
Refrigerator thermometers registering above 41ºF (e.g., resident room #312 refrigerator at 55ºF).
Building, equipment, and furniture not kept clean/in good repair: dirty/stained carpeting; partially disassembled aquarium left accessible to residents; damaged weather stripping on courtyard door.
Administrator stepped down from position and new appointment was not made.
Meals were being served to resident's guests/visitors when purchased, contrary to the facility's attestation letter.
Windows in memory care (MC) unit open more than 1/4 inch; at least two windows tilted in allowing full opening.
Insufficient supply of clean bed linens (towels, washcloths, and pillowcases) to meet the needs of a 93-bed home.
Staff did not have manufacturer's instructions or knowledge of proper cleaning chemicals for sanitizing the ice machine; pink mold observed on ice machine.
Potential fire safety violations: Assisted living exit doors used magnetic locks requiring staff access cards, and a courtyard fence gate was secured with a padlock that would not automatically release during a fire.
