The facility failed to ensure food items were protected from contamination by failing to properly label, date, and cover food in the refrigerator. Specific observations included multiple unlabeled containers of meat, rice, beans, and liquids, as well as uncovered pots and a corndog that appeared to have mold on it.

The facility failed to provide adequate supervision for a resident who walked unsupervised to a local fast-food restaurant. The resident did not sign out of the facility or notify staff of his departure, and was eventually returned to the facility by a police officer.
