The following deficiencies were found during the on-site compliance inspection conducted on November 22, 2024:
Based on record review and interview, the manager failed to ensure a personnel record for each employee included documentation of cardiopulmonary resuscitation training (CPR) and first aid training, for one of three employees sampled. The deficient practice posed a risk if an employee was unable to meet the needs of residents. Findings include: 1. A review of E3's personnel record revealed a CPR and first aid training certificate with a marked expiration of June 2024. However, documentation of current CPR and first aid training was not available for review. 2. In an interview, E1 and E2 acknowledged E3's documentation of CPR and first aid training was expired.
Based on observation, record review, and interview, the manager failed to ensure that medication was stored in a separate locked room, closet, cabinet, or self-contained unit used only for medication storage. Findings include: 1. During an environmental inspection of the facility, the Compliance Officer observed two boxes of Loperamide in an unsecured cabinet below the kitchen counter. 2. In an interview, E1 and E2 acknowledged medications stored by the assisted living facility had not been stored in a separate locked area.
Based on observation and interview, the manager failed to ensure a food menu was conspicuously posted at least one calendar day before the first meal on the food menu was served. Findings include: 1. During an environmental inspection of the facility, the Compliance Officer observed a white board included the food to be served on the day of the on-site inspection. However, a weekly menu posted at least one calendar day before the first meal on the menu would be served was not posted conspicuously. 2. In an interview, E1 and E2 acknowledged a food menu had not been conspicuously posted at least one calendar day before the first meal on the food menu was served.
Based on observation and interview, the manager failed to ensure poisonous or toxic materials were maintained in labeled containers in a locked area and inaccessible to residents. Findings include: 1. During an environmental inspection of the facility, the Compliance Officer observed a cabinet in the laundry room used for chemical storage had been left unlocked. Inside the cabinet, the Compliance Officer observed seven bottles of poisonous or toxic materials. 2. During an environmental inspection of the facility, the Compliance Officer observed a closet in the laundry room did not have a lock. Inside the closet, the Compliance Officer observed a tote containing poisonous or toxic cleaning chemicals. 3. In an interview, E1 and E2 acknowledged poisonous or toxic materials had not been maintained in a locked area and inaccessible to residents.
