The staff at this facility was incredibly skilled and attentive to my mom's needs . The wound care experts Clara and Dr margarita gave the best care to heal my mom's wound . Clara went over and above many times to keep it clean and healing and taught me to valuable information to help maintain this in the future if needed

Long-term resident's family · 2025★★★★★

My 80 y/o mom was hospitalized after a UTI. She couldn’t walk and needed help getting out of bed. After a week at Delray Medical Center she was improved enough to be discharged to Terrace of Delray. Each day she would deteriorate a little more. She was not eating without help. They did not help her eat. She ate very little each day.

This facility shows persistent care quality concerns across three surveys, with deficiencies in daily living assistance, pain management, and medication management recurring multiple times. One family filed a complaint regarding communication during care transitions, indicating real-world care coordination problems. While all 33 deficiencies across areas including fire safety, nutrition services, and resident rights have been corrected by the facility, the pattern of repeated issues in core care areas suggests families should carefully evaluate the facility's current practices during visits.
