Based on a review of resident records the facility failed to ensure that the facility shall report to the regional licensing office within 24 hours any major incident that has negatively affected or that threatens the life, health, safety, or welfare of any resident. Evidence: The record for Resident # 1 contained progress notes dated 5-27-25 that documented, ?Resident was found on the floor by her bedside. It appears resident hit her head. Bleeding was noticed on the left side of resident?s head. 911 was called.? The notes document that the resident was transported to the hospital. However, the facility failed to notify the regional licensing office of the incident. This was confirmed by staff.
