Hazardous and toxic chemicals were stored in unlocked kitchenette cabinets in the assisted living area, making them easily accessible to residents.
Multiple food items were found unlabeled in the assisted living common area kitchenette, activities cabinet, and the main service kitchen without appropriate open dates.
Garbage cans in the common area kitchenettes and learning center did not have tight-fitting lids.
Review of the dishwasher sanitization log for February 2025 revealed incomplete and/or blank entries of dishwasher sanitization temperatures.
