Key Review Excerpts

My wife received Skilled Nursing. Physical Therapy is Exceptional. The is a Physicians Assistant that is super. Extremely Knowledgeable about Medication. Nurses and CNAs are great.

Skilled Nursing family member · 2025★★★★★

The facility made it very easy for her to transition from the hospital to their facilities. She never missed a beat with her medications or physical therapy and quickly grew stronger.

Long-term resident's family · 2023★★★★★

Some of the nurses that helped her where nice, but the business was denying my mom her medicine. She was only there less and two weeks. She had to go back to the ER and ultimately having a heart attack and passing shortly after because her lack of treatment, as well as this just being an overall dirty, poorly ventilated, understaffed rehab facility.

$200,335 in fines

Families have filed multiple complaint reports against this facility citing serious concerns about abuse prevention and resident protection, with issues recurring across recent surveys from 2022 to 2025. The primary problem areas include protection from abuse and neglect, resident rights violations, and medication management deficiencies. While the facility has provided correction dates for all cited deficiencies, the pattern of complaint-triggered violations regarding fundamental safety protections should raise significant concerns for families considering placement.
