Observations revealed dirty floors needing sweeping and mopping, as well as dirt and grime buildup around toilets and in bathroom corners. Additionally, damaged tile was noted in a resident's room.

Specifically, the refrigerator contained several opened food items, including pimento cheese, jelly, salad dressing, and various bagged substances, that were not properly sealed, labeled, or documented with opened dates.
