The facility failed to provide the required minimum of 14 hours of planned group activities per week to promote socialization and physical interaction. Observations and interviews revealed no activity calendar was posted, and residents spent most of their time watching television or smoking. The Co-Administrator admitted that no activity calendar had been completed in over two months.

The facility failed to ensure therapeutic diets were served as ordered for a resident who required a mechanical soft diet. Specifically, the resident was observed being served a standard lunch consisting of a grilled ham and cheese sandwich and whole orange instead of the required modified texture. Additionally, the facility's kitchen records failed to accurately reflect the resident's prescribed mechanical soft diet.
