The facility failed to provide adequate supervision for a resident on a Special Care Unit in accordance with their care plan regarding choking hazards and falls. Specifically, the resident was observed chewing on a light bulb and puzzle pieces, and was found with a chewed shirt, indicating a failure to manage known risks. Additionally, there was no documentation that the physician or family were notified of these incidents.
