What this means for your family

This facility features a highly capable rehabilitation team that many families find beneficial for recovery. However, there is a consistent pattern of reports regarding severe understaffing and non-functional call bells; we strongly advise families to visit frequently and verify the responsiveness of the nursing staff during evening and weekend hours before committing.

Key Review Excerpts

The physical therapy is wonderful and very helpful. I got great care from Eric and Melissa, 2 of the nurses and Jayson the aid was great. There were times when I was in pain and it took a long time to get the medicine.

Rehab patient · 2026★★☆☆☆

My mother was a resident at Taconic since Nov of 2024. Thank goodness for the staff at the Taconic - they were all so helpful to me personally throughout her journey and more important, they showed my Mother kindness, patience and loving care up until her end just two weeks ago today.

Long-term resident's family · 2026★★★★★

The facility is an utter Hell Hole. Some of the staff are helpful but overall, they are so incredibly understaffed (and under-supplied) that no one gets the care and attention that is necessary to ensure a healthy outcome.
