The facility failed to ensure that one of three sampled medication aides had verified previous employment as a medication aide within the last 24 months or had completed the required state-approved training and competency evaluations. This represents a continuation of a previously identified non-compliance.

The facility failed to meet North Carolina Building Code requirements for non-ambulatory residents. Although licensed for 3 ambulatory residents, the facility housed 3 cognitively impaired residents who were unable to evacuate independently without assistance. Observations during a fire drill confirmed that residents did not respond to the smoke detector activation and required verbal coaching to evacuate.
