The facility underwent an unannounced Required Annual Inspection. The inspection found the facility to be clean, sanitary, and in good repair across all areas inspected. No deficiencies were cited during this annual review.

The facility underwent an unannounced case management visit. The Licensing Program Analyst found the facility to be in compliance with LIC808 and infection control plans. No deficiencies were observed during this visit.

The facility underwent an unannounced annual required inspection. The Licensing Program Analyst observed that the facility is in compliance with and has implemented necessary infection control practices as outlined in its COVID-19 Mitigation Plan. No deficiencies were observed during this visit.
