FACILITY EVALUATION REPORT

The inspection identified three deficiencies: one Type A related to outdated water supply testing, one Type A concerning unsecured cleaning chemicals, and one Type B regarding overdue annual medical reevaluations. The facility was noted to have addressed some immediate concerns, but the critical nature of the water and chemical storage issues requires prompt correction.

The facility underwent an unannounced Required 1 Year Inspection Visit. The inspector observed the facility to be clean, in good repair, and noted sufficient supplies and proper procedures for medication storage and emergency drills. No deficiencies were observed or cited per California Code of Regulations, Title 22 Division 6, Chapter 8.

The inspection was a required annual visit conducted on May 26, 2023. Two deficiencies were cited: one regarding the improper storage and pre-pouring of resident medications, and another concerning outdated Needs and Service Plans for residents. Both deficiencies are classified as Type B, requiring corrective action by specified due dates.

The facility was inspected for a case management review following an incident. The Licensing Program Analyst interviewed the licensee and reviewed records regarding an unresponsive resident. No deficiencies were cited as a result of this visit.
