The inspection was conducted unannounced and covered physical plant, client files, and medication review. The facility was found to be clean, in good repair, and most required documentation was up-to-date. No deficiencies were cited during this review.

This was a follow-up inspection prompted by deficiencies noted on previous visits regarding staff conduct, fire safety, and capacity limits. Multiple deficiencies were cited, including issues with staff sleeping in common areas, improper gate latching, and the need to secure updated fire clearance documentation. The facility was provided technical assistance on several key areas, including gate safety and fire clearance compliance.

The inspection identified two primary deficiencies: a Type A violation regarding fire clearance due to improper staff sleeping arrangements and facility layout discrepancies, and a Type B violation concerning the administrator's qualifications. Both deficiencies relate to safety and facility compliance, requiring immediate corrective action.

The unannounced visit found the residents to be in good health with no immediate safety concerns. However, the facility was cited for administrative deficiencies, specifically using the incorrect form for resident appraisals. Furthermore, the facility must submit updated administrative and personnel reports by a specified deadline.

This was an unannounced POC visit conducted to verify the correction of citations from a previous case management visit on 2/21/2023. The report indicates that the facility has cleared all deficiencies cited under Title 22 Regulations and complied with the terms of the Plan of Correction (POC) by the due date.

The inspection revealed several deficiencies, including immediate safety risks related to fire equipment and disaster drills. Additionally, there is a non-compliance issue regarding the required annual medical assessments and documentation for residents with dementia. Corrective actions are required for all cited violations.
